To register at the practice please ask at reception. If you already have a GP in the area you may be invited to tell us why you want to register with us - it helps us to better understand our patients' needs. The receptionist will provide you with a registration form, or you can download a copy below, which must be completed before registration can be achieved, please also bring photo ID and proof of your home address.
Medical treatment is available from the date of registration. If you move, or change your telephone number, please let us know so we can update your records. You will need to find another doctor if you move outside our practice area. We reserve the right to cancel the registration of any patient who is violent or abusive towards our staff.
All patients over 75 and all new and existing patients will be given the name of your accountable GP on your next visit. Please enquire if you would like this information in advance of your next appointment. The accountable GP will be ultimately responsible for your care, but this does not stop you seeing any GP on the day.
